This an English copy of the Finnish RC-report form. It can be sent to the RC as an attachment or by post. Presumably it can be used to report birds in any other country as well. Download it and change the headings if needed. Lines can be added to allow for as much text as is suitable (especially the description fields and the observer field will be more lines).

The report is rtf-formatted (Rich Text Format) and can be used in Microsoft Windows 95/98/NT with MS Word text processing program. If you want to fill it in by some other means, (not in MS Word), first add lines to make the form one-page long, before you print it.
